Documents and payment

The duly filled application form should be submitted together with the original bank slip / bank deposit slip of Rs. 1,000/-, paid to Account No: 048-1-001-8-0086726 at any branch of People’s Bank, in favour of SLGTI, Ariviyal Nagar, Kilinochchi, Sri Lanka.

Level 04: Complete either this O/L block or all NVQ fields in step 5 (you may do both). NVQ-only applicants may leave index, year, and all O/L results empty. If you enter index, year, or any result here, finish the whole section — including a letter grade (A–W) in each “Result” box for all six main subjects and for all three basket subjects you choose.

Step 7 — Documents

Upload NIC copy, birth certificate, and bank slip (PDF, JPG, or PNG, max 5 MB each). Upload an O/L certificate if you completed O/L in step 4, and an NVQ certificate (or N/A scan) if you completed NVQ in step 5. Images and raster PDFs are stored as compressed JPEG under 100 KB; if the server cannot convert a PDF, the original PDF is stored instead.
